[Openswan dev] [Patch] fix for pluto 'nhelpers > 1' crash

Initially ran into this issue with 2.4.x. From memory, the ikev1 
continuation was segfaulting usually due to a NULL state, because only 
the first helper's continuations were cleaned up properly (eg. on dpd, 
sa expires..)
